New Patient Registration

 The doctors welcome new patients who live within our practice area. 

If you live in our practice area (see below) and you are entitled to use the NHS, you can register with us.

Come into the Health Centre with two forms of identification. These can include:

  • birth certificate
  • marriage certificate
  • medical card
  • driving licence
  • passport
  • local authority rent card
  • paid utility bills
  • bank/building society cards/statements
  • National Insurance number card
  • payslip
  • letter from Benefits Agency/benefit book/signing on card
  • papers from the home office
  • P45


We will ask you to complete a health questionnaire so that we have as up to date information as possible. We will also invite you to see a GP or nurse for a health check, during which you can agree a Health Plan and take a printed copy of it with you.

You will be registered with the Practice rather than a specific doctor, but you still have the right to ask to see a particular doctor, if you wish. If you have a repeat prescription then you should arrange an appointment with the doctor as soon as possible, so that we can add the details into our computer system.

We do not exclude patients on grounds of race, sexuality, religion, social class, age, disability or medical condition.

Practice Boundary

Our practice area is bounded on the west by Chapel Park, Newbiggin Hall and Kenton Bankfoot, in the north by Kingston Park Road and in the east by Great North Road and Central Motorway. In the south by Mosley Street across to Scotswood Road, up the Western Bypass to Denton Burn, then west along the A69 as far as Chapel Park.
